General Information About Choosing Your Cosmetic Surgeon

Despite the fact that cosmetic surgery is more predictable than ever before, it is still a serious undertaking. Be sure that you have all the facts to make an informed choice. No aspect of your decision is more critical than selecting the proper surgeon. Qualified cosmetic surgeons will welcome your questions about their background and be pleased that you are concerned enough to ask.

Guidelines in Your Search

Here are guidelines to assist you in selecting a qualified cosmetic surgeon:

Check into the surgical experience of the surgeon you are considering. Does he or she specialize in the procedure(s) in which you are interested? How many procedures of this kind has the doctor done? How many is the doctor currently performing per year? Before-and-after photographs can give you some indication of a surgeon's ability, although you must realize that they cannot be construed to guarantee the result you will achieve. Make sure you are comfortable with the personal support between you and your surgeon. In addition, you should feel at ease with the staff. You should always feel that your concerns are being addressed. Do not be satisfied with an incomplete answer from anyone. Asking About Credentials

If board certification is to be used as a yardstick in determining a surgeon's qualifications, it is important to ask your doctor about his or her credentials and study them carefully. Initial certification by one of the member boards of the American Board of Medical Specialties (ABMS), such as the American Boards of Dermatology, Otolaryngology, Ophthalmology, General Surgery or Plastic Surgery provide some assurance of formal training in the fundamentals of cosmetic surgery.

Some board-certified physicians will then go on to become-board certified by the American Board of Cosmetic Surgery, which has established a strict set of criteria to ensure experience and proficiency in cosmetic surgery. A physician certified by the American Board of Cosmetic Surgery must:

Be board-certified in an original surgical specialty recognized by the American Board of Medical Specialties.

Have been in the practice of cosmetic surgery for the past five years.

Have performed no fewer than 1,000 cosmetic surgical procedures.

Be currently performing a minimum of 200 documented cosmetic procedures per year.

Pass a stringent two-day oral and written examination.

Cosmetic Surgery:  Plastic Surgery and Total Patient Care

For cost-containment and convenience, an increasing number of procedures — especially cosmetic operations — are being performed in freestanding or office-based surgical facilities on an outpatient basis. Patients planning to have surgery in this type of facility should ensure that it is properly equipped and staffed, that it has access to a nearby hospital, that the anesthesia will be administered by a well-trained professional, and that the doctor has privileges to perform the same procedure at an accredited hospital.

One additional sign of a quality facility is accreditation by the American Association for Accreditation of Ambulatory Plastic Surgery Facilities (AAAAPSF). This organization not only inspects the facility itself, it requires that the practicing physicians are certified in plastic surgery and have operating privileges at a local accredited hospital. AAAAPSF re-inspects facilities every three years. A patient can check if a plastic surgeon's facility is accredited by phoning ASPS at 1-800-635-0635.

Cosmetic Surgery:  Plastic Surgery and Total Patient Care

Filler implant insertions typically take up to an hour to perform in the office, depending upon the number and extent of treatment locations. Several sutures are used to close the insertion site.

More swelling redness, and discomfort may accompany these procedures, which require the use of a local anesthetic. Local application of cold compresses may feel comforting.

Most early swelling subsides within a day or two, though minor swelling and irritation may last for several weeks during the implant healing phase. Your activity level and medication for pain after the procedure are dependent upon your doctor's recommendations.

Insurance does not generally cover surgery that is purely for cosmetic reasons. Surgery to correct or improve facial scars or contour deformities resulting from injury may be covered. It is the patient's responsibility to check with the insurance carrier for information on the degree of coverage.

Cosmetic Surgery:  What are the risks and how can I find the right doctor for me?

The first laser was approved for LASIK eye surgery in 1998. Therefore, the long-term safety and effectiveness of LASIK surgery is not known. Additional Risks if you are Considering the Following:

Monovision:   Monovision is one clinical technique used to deal with the correction of presbyopia, the gradual loss of the ability of the eye to change focus for close-up tasks that progresses with age.

The intent of monovision is for the presbyopic patient to use one eye for distance viewing and one eye for near viewing. This practice was first applied to fit contact lens wearers and more recently to LASIK and other refractive surgeries.

With contact lenses, a presbyopic patient has one eye fit with a contact lens to correct distance vision, and the other eye fit with a contact lens to correct near vision.

Cosmetic Surgery:  Rhinoplasty-The Nose Job

You can expect a thorough explanation of the surgeon's expectations and the risks involved in surgery.

Following a joint decision by you and your surgeon to proceed with rhinoplasty, the surgeon will take photographs of you and discuss the options available.

Your surgeon will explain how the nasal structures, including bone and cartilage, can be sculpted to reshape the nose and indicate how reshaping the chin, for example, could enhance the desired results.

Cosmetic Surgery:  Rhinoplasty-The Nose Job

The definition of rhinoplasty is, literally, shaping the nose. First, incisions are made and the bone and cartilage support system of the nose is accessed.

The majority of incisions are made inside the nose, where they are invisible. In some cases, an incision is made in the area of skin separating the nostrils. Next, certain amounts of underlying bone and cartilage are removed, added to, or rearranged to provide a newly shaped structure.

For example, when the tip of the nose is too large, the surgeon can sculpt the cartilage in this area to reduce it in size. The angle of the nose in relation to the upper lip can be altered for a more youthful look or to correct a distortion.

The tissues are then redraped over the new frame and the incisions are closed. A splint is applied to the outside of the nose to help retain the new shape while the nose heals. Soft, absorbent material may be used inside the nose to maintain stability along the dividing wall of the air passages called the septum. Alternatively, soft nasal supports that permit nasal breathing post-operatively can be placed.

Cosmetic Surgery:  All About Breast Implants

Silicone Gel-Filled Breast Implants:   The three types of silicone gel-filled breast implants are as follows:

One type is a single lumen implant that is pre-filled by the manufacturer with a fixed volume of silicone gel.

A second type is a double lumen implant with (1) an inner lumen pre-filled by the manufacturer with a fixed volume of silicone gel and (2) an outer lumen that is filled during the operation with a fixed volume of saline through a valve.

A third type is a double lumen implant with (1) an outer lumen prefilled by the manufacturer with a fixed volume of silicone gel and (2) an inner lumen that is filled during the operation with saline through a valve. This type of implant allows for adjustments of the saline volume after the operation. A silicone gel-filled breast implant has a silicone rubber shell with the same general composition as shown in the Saline-Filled Breast Implants section above.
